As a Family Resource Coordinator at Special Kids, you will have the opportunity to work alongside individuals who are passionate about serving others. You will be responsible for processing patient referrals, assisting families in navigating all aspects of social services, assisting families with grant applications, and assisting the Lead Family Resources Coordinator in providing parent education classes.

JOb DETAILS

  • Complete initial intakes with families that have been referred for services.

  • Process patient referrals and obtains all necessary information and documentation for patient files such as medical records.

  • Assist families in navigating all aspects of social services that are available to them such as school services, grant funding, etc.

  • Assist families with grant applications.

  • Refer families for outside community resources and to other organizations as necessary.

  • Provide crisis management for families as necessary.

  • Act as consultant to educational, medical, and other professional groups as well as fellow staff members.

  • Conduct individual family meetings as necessary.

  • Monitor staffing needs of the Family Resources department as well as all Family Resources caseloads and referrals and submits the necessary paperwork for new position requests based on those needs.

  • Gather and submits department data to the Director of Therapy for monthly productivity reports.

EXPERIENCE

  • Master’s degree preferred or Bachelor’s degree from four year college or university in Social Work or other related field.

  • None required; LBSW preferred, CPR certification recommended

OTHER SKILLS

  • Knowledge of Microsoft Office software.  Working knowledge of HIPAA and electronic medical records is a plus.

  • To perform this job successfully, an individual should have basic computer skills and the ability to learn new software and web-based applications such as electronic medical records.
